School Forms + Parent Information, 2023
Please note that we aim to make this as easy for all of our families as possible; however, New York State requires forms are filled out and signed by parents each year (some also by health care providers), and that we have hard-copy forms on-file at all times. Therefore, all forms must be returned in HARD COPY form by August 30. Thank you for your support!

REQUIRED Forms for ALL CHILDREN
Child in Care Medical Statement (including all NYS required vaccines - COVID-19 and flu vaccines also strongly recommended in a preschool setting; health care provider signature required)
IF YOUR CHILD HAS ANY INDIVIDUAL NEEDS
The following are required for children with needs related to allergies, health, behavorials, or special education/early intervention:
Individual Health Care Plan Form (required for all children with IEPs, services, allergies, medical considerations or additional special needs)
Individual Allergy and Anaphylaxis Emergency Plan and Action Card (required for all children with allergies, health care provider signature required)
Medication Consent Form (only complete if your child requires emergency medication - inhaler, epi-pen, etc. - health care provider signature required)
Parent Consent Form (required if your child will receive services from a therapist/specialist on-site)

> Drop off at the Temple during office hours.
> Drop off in the milkbox next to the Temple south side door, in a sealed envelope labeled "Nursery School Forms".
